To nominate an individual for the 2016 Dr. Martin Luther King, Jr. Youth Service Award, please complete this form and return it to the King Nomination Committee with a photo, bio, the reasons why the nominee should be honored, and evidence of community service.  Submissions must be postmarked by December 4, 2015.  The Youth Service Award is limited to individuals residing in Southwest Georgia.  Service may be in the form of leadership of social, economic or political change and/or ministry to the economically or socially disadvantaged.

One Individual Youth Service Award will be presented based on the following criteria:

This youth:

            *must be between 12-17 years of age;

            *must reside in Southwest Ga. and is enrolled in a public or private school in Southwest    Georgia;            

            *must be in good academic standing;

            *must be of high moral character;

            *is known to have inspired peers and adults by using his/her talents and leadership;

            *is motivated by genuine concern for his/her fellow man; and
