To nominate an individual for the 2016 Dr. Martin Luther King, Jr. Dream Award, please complete this form and return it to the King Nomination Committee with a photo, bio, the reasons why the nominee should be honored, and evidence of community service. Submissions must be postmarked by December 4, 2015.  The Dream Award is limited to individuals residing in Southwest Georgia.  Service may be in the form of leadership of social, economic or political change and/or personal ministry to the economically or socially disadvantaged.

Criteria for the Selection of the Dream Awards are:

This person:

*demonstrates the qualities of a servant leader;

*acts with integrity, faith and confidence on behalf of something that has been committed to his/her care;

*acts as an advocate for those who are less fortunate;

*seeks to serve first then lead;

*has a history of community service and demonstrates attributes of trust by strong service and leadership  on behalf of the whole and for the common good;

*inspires and influences others by using his or her gifts, skills and energies;

*is motivated by genuine concern for his/her fellow man rather than personal aggrandizement;

*is a good steward of the community and its resources;

*is an agent of change for the common good;

*listens to the diverse voices of the community but is willing to take up a contradictory position;                 

*empowers others to grow;

*generously shares time, talent and energy in helping others to a better life;

*envisions a better world and is committed to achieving that vision; and

*resides in Southwest Georgia.
