ALBANY, GA (WALB) -

Colquitt Regional Medical Center is handing out free Critical Conditions guides to adults ages 18 and up for National Healthcare Decisions day. 

Hospitals throughout the US are using the day as an initiative to encourage adults of all ages to think about what medical decisions they want to be made for them in the event they cannot make the decisions themselves.

The guide includes conversation starters for families and forms needed in preparing advanced directives.

An advanced directive is a legal document for healthcare wishes so that family members don't have to make difficult decisions based on what they assume you would want during the final stages of life.

Healthcare officials say there are a few options for the legal documentation, including healthcare power of attorney to choose someone to make any medical decisions needed.

DNR orders state that you don't want to be kept alive in case of a heart attack or breathing stops and living wills help preplan medical treatments.

Advanced directives are available with doctors, hospitals, or state agencies on aging.
